The Pritzker School of Medicine, through Chicago Street Medicine, is offering free home visits by a UChicago physician for adults (18+) who face barriers to accessing care due to concerns about ICE.
Eligible ZIP codes: 60608, 60623, 60637, 60615, 60619, 60653
Languages: English & Spanish
Services include: Vitals checks, basic wound care, blood pressure and diabetes screening, and select OTC medications. To request a consultation, submit the form or email csmhomevisitprogram@gmail.com for the link.
